This is a 52-year old man who was concerned with the excess skin along his upper eyelids and puffiness along his lower eyelids. His medical history was significant for dry eyes. Upon physical examination he had excess skin of the upper eyelids and protrusion of the fat pads along both the upper and lower eyelids. The patient was considered a good candidate for a bilateral upper and bilateral lower blepharoplasty. For the upper eyelids, Dr. Sweis removed the excess skin, a small strip of muscle and a minimal amount of fat from the medial and central fat pads that rest along the upper aspect of the orbit. For the lower eyelids, the incision was made inside the lower lid (transconjunctival blepharoplasty), the protruding fat was repositioned along the tear trough to address some of the hollowness, with only a minimal amount of fat removed from the 3 fat pads. There was no skin excision from the lower eyelids to avoid even the slightest downward shift of the lower eyelid position (scleral show or ectropion) which would worsen his dry eye condition. His pre-operative photos are shown along with his photos 3 months following his procedure.
