This is a 53 year-old woman who presented to Dr. Sweis complaining of having excess skin along the upper eyelids and puffiness of the lower lids. Upon physical examination she was found to have excess lax skin (dermatochalasis) of both the upper and lower eyelids, relaxation (pseudoherniation) of the medial and central fat pads along the upper eyelid as well as pseudoherniation of the medial, central and lateral fat pads of the lower eyelids. After discussing her options, the patient decided to proceed with a bilateral upper Eye Lid Lift and a bilateral lower Eye Lid Lift. For the lower eyelids, the incision was made inside the lids (transconjunctival) to address the fat pockets. A minimal amount of fat was removed from the three lower lid fat pads and repositioned along the tear trough.

The excess skin of the lower eyelids was removed through a separate skin incision just underneath the lash line. This approach does not interrupt the lower lid muscle and releases the structures that could potentially pull the eyelid down, thereby minimizing the potential for changing the shape of the eyes. Her photos are six months after surgery.
